cjkas 04cb97e3bf docs: plan auth on every entry point and one HTTP server 
SECURITY_PLAN.md records the agreed design: an access table for port 80
and the 8081 alias, websocket handshake auth, CORS removal, a random
token key, /login throttling, and moving API and websocket to port 80.
KNOWN_ISSUES.md gets an entry for each open issue it fixes.
